Requirements: active CPR certification (within the last 12 months); clean drug test (within the last 12 months); clean criminal background report (within the last 12 months); current immunization records; current negative TB test (within the last 12 months); two government-issued personal identification cards: (current driver’s license and social security card); current phone number and dependable transportation
