Active CPR certification (within the last 12 months), clean drug test (within the last 12 months), clean criminal background report (within the last 12 months), current immunization records, current negative TB test (within the last 12 months), two government-issued personal identification cards: (current driver’s license and social security card), and current phone number and dependable transportation. Ideally, prefer current certified nursing certification.
